Cold continental breakfast served in the room is a grey area, but in most B&B contexts it’s still standard-rated as it’s supplied as part of an accommodation service. The threshold for VAT registration is £90,000 in turnover in any 12-month period — many York B&Bs with a handful of rooms are above this and need to register. If you’re not registered and should be, HMRC will assess the VAT you should have collected, with interest.
